Dialysis Access Maintenance Coding - A Closer Look for 2010

Arteriovenous fistulas (AVF) are surgically created conduits, which directly connect a superficial artery to a venous structure. Often these grafts become occluded and require a therapeutic intervention to restore optimal function to the graft.  Most commonly AV grafts are treated by percutaneous angioplasty and/or thrombectomy and on some occasions the physician may elect to place a stent.

Fistulagram 

At the beginning of the procedure the physician will most often gain access into the AV graft by a direct puncture into the graft. Once the physician gains access he or she will place the catheter at the desired position and then inject contrast material to visualize where the graft may be occluded. This imaging procedure is referred to as a fistulagram. Direct access into the graft with performance of fistulagram is described by CPT code 36147. Effective January 1, 2010 code 36147 replaced codes 36145 and 75790. Imaging of the graft is now bundled into the surgical component when it is performed via a direct puncture into the graft. Repeat fistulagrams are not reported separately. Note that code 36147 includes imaging all the way to the inferior or superior vena cava.

There may be times when the physician needs to enter the graft from the opposite end of the original access to facilitate performance of a therapeutic intervention. The second access required to perform the therapeutic intervention is reported with add on code 36148. Code 75791 will be assigned for a fistulagram ONLY when it is performed through an already existing access into the shunt or from an access that is not a direct puncture of the shunt. Use of this code should be rare. The following is a list of the applicable codes and their full descriptors for your reference. 


Access & Fistulagram Codes

36147 Introduction of needle and/or catheter, arteriovenous shunt created for dialysis (graft/fistula); initial access with complete radiological evaluation of dialysis access, including fluoroscopy, image documentation and report (includes access of shunt, injection[s] of contrast, and all necessary imaging from the arterial anastomosis and adjacent artery through entire venous outflow including the inferior or superior vena cava)

(If 36147 indicates the need for a therapeutic intervention requiring a second catheterization of the shunt, use 36148)

(Do not report 36147 in conjunction with 75791)

+36148 additional access for therapeutic intervention (List separately in addition to code for primary procedure)

(Use 36148 in conjunction with 36147) 

75791 Angiography, arteriovenous shunt (eg, dialysis patient fistula/graft), complete evaluation of dialysis access, including fluoroscopy, image documentation and report (includes injections of contrast and all necessary imaging from the arterial anastomosis and adjacent artery through entire venous outflow including the inferior or superior vena cava), radiological  supervision and interpretation

(Do not report 75791 in conjunction with 36147, 36148)

(Use 75791 only if radiological evaluation is performed through an already existing access into the shunt or from an access that is not a direct puncture of the shunt)

Angioplasty

A common treatment option for an occlusion of an AV graft is angioplasty. Percutaneous transluminal angioplasty is performed to eliminate areas of narrowing or occlusion in the graft. During the PTA a balloon catheter is inserted through the skin into a vessel to the site of narrowing.

For 2010, HCPCS G0392 for angioplasty at the arterial anastomosis and G0393 for angioplasty at the venous anastomosis were deleted. PTA of the graft now should be reported with CPT codes 35476/75978 for the venous anastomosis or 35475/75962 for the arterial anastomosis.

Only one PTA may be reported for the entire graft since the entire graft is considered one vessel for coding purposes. It is important to note that CPT code 35476 is a column 1 code to 35475 in the NCCI edits, which is the opposite of the G codes– G0392 was made a column 1 code to G0393 last year. If a PTA is performed at both the arterial and venous anastomosis, this should be reported as a single venous PTA, unless otherwise specified by a particular payer. The Society of Interventional Radiology has advised (for coding purposes) that the AV graft is comprised of the inflow artery at the arterial anastomosis, the arterial anastomosis, through the entire length of the graft to the venous anastomosis, all the way through the venous outflow to the level of the axillary vein.

Therefore, all PTA within this segment should be coded as a single venous PTA, regardless of the number of stenoses treated within the segment. Angioplasty coding conventions outlined in CPT hold true for angioplasty of an AV graft. Only one angioplasty code is assigned when multiple stenoses are treated in the same vessel or graft. Angioplasty may be coded a second time when a separate stenosis outside the graft is treated in a central vessel such as the subclavian. The need for treatment of the separate stenosis should be clearly documented in the report. If an angioplasty is performed of the subclavian vein, codes 35476 and 75978 may be coded for the angioplasty of the subclavian in addition to the angioplasty of the graft. The -59 modifier will need to be appended to both of these codes to indicate that the angioplasty is separate and distinct from the angioplasty of the AV graft. The following is a list of the applicable codes and their full descriptors for your reference. 


Angioplasty Codes 

35475 Transluminal balloon angioplasty, percutaneous, brachiocephalic trunk or branches, each vessel

75962 Transluminal balloon angioplasty, peripheral artery, RS&I 

35476 Transluminal balloon angioplasty, percutaneous, venous 

75978 Transluminal balloon angioplasty, venous (eg, subclavian stenosis), RS&I


Thrombectomy

Sometimes the physician may opt to perform a thrombectomy to remove an occlusion. A thrombectomy is a treatment that uses an endovascular device such as an AngioJet or Trerotola, to fragment and remove stenoses in occluded vessels.

A thrombectomy may be the only therapeutic intervention that is performed, however in many instances a thrombectomy is performed during the same session as angioplasty. Code 36870 is used to describe percutaneous thrombus removal from the AVF. This code includes only the work necessary to remove the thrombus from the AVF, including all mechanical and pharmacological means of thrombus removal.

It is notable that there is no corresponding RS&I code for 36870. Code 36870 should be reported in addition to the other codes that describe the procedure. In the event there is a thrombus outside of the graft, and thrombolysis (infusion of a drug to dissolve a clot) is performed, codes 37201 and 75896 would also be assigned for the thrombolytic infusion. Many times the physician will utilize Heparin during a thrombectomy procedure. This is NOT considered thrombolysis and should not be coded as such. The Heparin is used to keep the vessel patent during the procedure. The following is a list of the applicable codes and their full descriptors for your reference.

Conclusion

Although the information in this article should have you well on your way to successfully coding these procedures, it is important to realize that with these procedures as with all interventional procedures, there may be variations in how a physician performs a procedure as well as variations in anatomy that may affect coding. Coders should always take care to read all reports thoroughly to ensure that all codes that describe the work performed are captured and reported accurately.
